Social Worker (MSW)

Position is responsible for delivery of varied social work services to hospice patients and families, including initial psychosocial evaluations, ongoing psychosocial counseling, direct casework services, and bereavement services.  This position is full-time and offers an excellent benefit package including fully paid medical, dental, short/long term disability, and up to 25 days of paid time off.

Requirements:
– Master’s Degree of Social Work from school accredited/approved by the Council on Social Work Education
– Certified Social Work Associate (CSWA), in the state of Oregon (or ability to become certified)
– A demonstrated ability in casework, counseling and group work, including clinical social work experience preferably in a healthcare or hospice setting
– Computer literacy with Microsoft Office programs
– Possess valid state driver’s license and reliable automobile, and be willing to operate personal car as necessitated by nature of job

Supplemental RNs

The Hospice Supplemental Registered Nurse is responsible for providing and documenting care for terminally ill patients from day of admission through day of discharge.  Utilizing physicians’ orders and professional skills, and in coordination with the other members of the interdisciplinary team, the Hospice Supplemental RN implements a hospice plan of care that meets each patient’s specific needs and is in compliance with Federal and State regulations, reimbursement guidelines, as well as the organization’s policies and procedures.  Participates in on-call rotation (including weeknights and weekends).

Requirements:
– Graduate of an accredited school of nursing
– Current RN license from the Oregon State Board of Nursing, with the ability to obtain a RN license from the California Board of Registered Nursing.
– Experience as an RN commensurate with ONE of the following: one year acute care experience within the last three years; one year home health care experience within the last three years; one year community health or public health experience within the last three years; one year hospice experience within the last three years; or, have a baccalaureate degree in nursing from a program accredited by the National League of Nursing.
– Current BCLS certification
– Basic computer knowledge
– Comprehensive knowledge of general nursing theory and practice
